Alonso Family Dental Membership Plan
Alonso Family Dental now offers an in house dental plan, which all patients are eligible for. By joining our dental membership, you acknowledge that services rendered while under your individual chosen membership plan, will NOT be filed to any insurance companies. Unlike insurance plans, these membership plans do NOT have a deductible or maximum per year. Memberships do require payment in full at the time of sign up and are valid one year from date of the membership agreement. 10% off applies to any dental treatments and payment is required in full on the day of treatment or prior to treatment if one wishes.

$600 annual membership (adult)
$500 annual membership (child)
- 1 basic cleaning every 6 months
- Oral cancer screening with cleaning
- 1 new patient comprehensive exam
- 1 follow up exam per year
- X-rays as determined necessary
- 1 emergency exam with X-ray/year
- 10% off any needed treatment
Adult Preventive Plan$500 annual membership
- 1 basic cleaning every 6 months
- Oral cancer screening with cleaning
- 1-2 exams/year
- X-rays as determined necessary
- 1 emergency exam with X-ray/year
- 10% off any needed treatment
Child Preventive Plan$400 annual membership
- 1 basic cleaning every 6 months
- Oral cancer screening with cleaning
- 1-2 exams/year
- X-rays as determined necessary
- 1 emergency exam with X-ray/year
- 1 Fluoride treatment/year
- 10% off any needed treatment
Period Maintenance Plan$800 annual membership
- 1 periodontal cleaning every 4 months
- 1-2 exams/year
- X-rays as determined necessary by your dentist
- 1 emergency exam with X-ray/year
- Oral cancer screenings
- 10% off any needed treatment
